Florida Department of Health requires all minors have a notarized consent form completed in order to receive a body piercing.
Parental consent is required by law for a minor to receive a body piercing, no exceptions.
Parent or legal guardian (with proof of guardianship) and minor child must both have their legal forms of identification present to receive a body piercing along with the notarized consent form completed.
If you feel that all of these requirements are too much to ask to have your minor child pierced take your issue up with someone other than us!
